Upon receipt of this application, the City will notify the designated contact person for the applicable public access area advising of the application.

Your application will be reviewed by the City of Okoboji City Council at their next regularly scheduled City Council meeting.

All dock and hoist privileges shall expire on December 31 of each calendar year until renewed.

All dock and hoist privileges shall be renewed on an annual basis. The annual application period shall be open until March 15. Any applications submitted after March 15 shall be considered for the FOLLOWING calendar year.

The fees for dock and hoist privileges are established by Resolution as part of the City of Okoboji Fee Schedule.

Applicants shall maintain liability insurance in the amount not less than One Million Dollars ($1,000,000) and shall provide proof of such coverage annually.

Applicants shall require their insurance carrier to name the City as an additional insured (not merely a certificate holder). The Applicant shall provide proof of this additional insured status to the City. Naming the City as an additional insured does not waive any governmental defense/immunity available to the City under Iowa Code Chapter 670 or otherwise.

I understand and agree that commercial use of this dock is prohibited. This includes that no hoist shall be leased, rented, sublicensed, assigned or otherwise provided for the benefit of another person.I understand that this application through the City of Okoboji does not serve as a comprehensive authorization for compliance with the State of Iowa DNR. All additional mandates and prerequisites stipulated by the Iowa DNR must be fulfilled independently.I understand and agree that I will abide by all pertinent State Codes governing dock regulations.I understand that failure to adhere to any of the aforementioned terms or the regulations governing public lake access, as defined in Chapter 147 of the City of Okoboji Code, may lead to revocation of this privilege.I understand that the Public Access points are City of Okoboji property and that the general public shall have the right to use any public access area and any dock extending therefrom, except for boat hoists, provided such use does not unreasonably interfere with rights of persons holding dock or hoist privileges.I have fully read and understand Chapter 147 of the City of Okoboji Municipal Code and agree to abide by all of the terms within the ordinance
